STEP 1 – PLANNING DOCUMENTS
Begin by downloading the Personalized Learning Plan Template. It will be useful for you to have in front of you as you watch the videos. This Excel document is where you will write your plan.
Download and save as “School Year – Name of Your School – Personalized Learning Plan.” For example, if your school is Clark High School then save the document as 2013 – 2014 Clark High School Personalized Learning Plan. This will help kid∙FRIENDLy staff manage more than 100 Personalized Learning Plans!
Personalized Learning Plan Video 3 Script
These notes accompany Video Series 3 and contain the transcript of the video. This is helpful to have while viewing the videos and may be referenced later during actual planning.
District Request for Technology Funds
This document should be completed by the superintendent or his/her designee. Instructions are available in Video 3 – Section 9.
Family Resource:Youth Service Center (FRYSC) Request for Funds
This document should be completed by the FYRSC Director and principal. Instructions are available in Video 3 – Section 9.
FRYSC Activity Funds Record
This document should be used throughout the school year to maintain a record of expenditures and resulting impact. Instructions for completing this form and final submission of the documents are available in Video 3 – Section 9.

STEP 3 – VIDEO 2 PERSONALIZED LEARNING PLANS – COMPLETING TWO TECHNOLOGY SURVEYS
There are two required surveys. The Technology Survey should be taken by one member of the Personalized Learning Team after the Team has collectively determined the response to each item. The kid∙FRIENDLy Data Collection Survey should be completed only by the principal. This video provides an overview of the Technology Survey.
